Mailing Instructions

 

• To receive your CD-ROM set, please send us your DVD and the plastic jewel case to the address below.   Only the DVD and jewel case are required from your product box.  You may keep all other packaging materials.  

• Fill in the information below, print and include in your mailing. (If you don’t have a printer, you may write the information on a blank piece of paper.)

 

• Mail the DVD, plastic jewel case and this form via the United States Postal Service.  Mail your materials using the service “Media Mail with Delivery Confirmation.”  This is a cost-efficient way to mail back your materials and provides you with the date and time your materials were received by us.           

 

• Once your DVD has been received and verified by us, we will ship out the CD-ROM replacements.
